Dear Parents,

All Catholic schools in the Salt Lake Diocese will be utilizing the "FACTS Grant & Aid Assessment Program", a third party financial needs evaluator, for the 2013-2014 school year. This assessment will aid Kearns-ST. Ann School in determining eligible families who will receive tuition assistance from Kearns-St. Ann School. Families applying for financial assistance must complete an application and submit necessary supporting documentation to FACTS. Paper applications are available at school. Applicants my also apply through the Internet at www.factstuitionaid.com. ALL FAMILIES REQUESTING TUITION ASSISTANCE MUST APPLY THROUGH FACTS USING THE APPLICATION PACKET OR THE INTERNET.

Please mail the following information to FACTS.

  1. Signed and completed paper application
  2. Application fee; check payable to FACTS
  3. Copies of your most recent tax forms including all supporting schedules
  4. Copies of your 2012 W-2 forms for both you and your spouse
  5. Copies of supporting documentation for Social Security Income, Welfare, Child Support, Food Stamps, Workers compensation and TANF.

Failure to submit all of the necessary information to FACTS by the deadline may result in your family not receiving financial assistance or limit the amount given due to incomplete information.
